EXTENDED ILLNESS TIME (EIT)
Eligible team members accrue 2.154 hours of extended illness time per pay period up to a maximum of 480 hours. Accrual begins after 90 days employment.

MEDICAL INSURANCE
Team members will choose either a PPO or an HRA plan. The PPO plan offers in-network options at the FMOLHS level and PPO level and includes out of network benefits. The HRA is a consumer driven option with high deductibles, an employer-funded health reimbursement account and lower premiums.

THE FRANCISCAN CLINIC
Medical Home for team members and families covered on Our Lady of the Lake's health plan. $5 co-pays.

ANNUAL HEALTH EXAM
An annual health exam is covered at 100% for participating team members and spouses participating in the health plan.

DENTAL INSURANCE
Provides 100% coverage for preventative services. The plan includes coverage for restorative services and also orthodontia for dependents to age 19. Since there is no network, participants may seek services from any provider.

VISION INSURANCE
Full service plan with allowances for exams, frames, and contact lenses.

LONG TERM DISABILITY
Benefits provide 50% of monthly salary (to a max of $3,000 per month) after 90 days of continuous total disability. Employees may purchase a buy-up option of 60% of monthly salary (to a max of $10,000 per month).

FLEXIBILE SPENDING ACCOUNT
